A short checklist so your first visit at TKM Labs runs smoothly — nothing fancy, just the things that let your Doctor of Physical Therapy hit the ground running.
Bring these documents
- Photo ID and insurance card (or your payment method if cash-pay).
- Referral or prescription if your provider sent one — Utah is direct-access, but the paperwork helps.
- Recent imaging or reports — MRI, X-ray, or surgical notes for the shoulder or elbow.
- A list of medications and any relevant medical history.
Wear something you can move in
For shoulder or elbow work we need to see the joint move. A tank top or loose t-shirt works great. Athletic shorts or leggings are perfect. Skip anything you'd be uncomfortable rolling up or removing for the exam.
Bring a few notes on your goals
The clearer you are about what "better" looks like — sleeping on the affected side, pressing overhead at the gym, throwing pain-free — the sharper we can make the plan.
Bring your questions
Write them down. "How long is this going to take?" "Can I keep training?" "Should I ice or heat?" We'll answer them all before you leave.
What you don't need to bring
You don't need to arrive early to fill out paperwork — we send it digitally ahead of time. You also don't need to be in perfect shape or symptom-free. Come as you are; we'll take it from there.
